Consultations are by appointment only.
Appointments may be made in person or by telephone. You can, if you wish, make an appointment to see any of the doctors.
Phoning early gives us the best chance of meeting your first choice for a routine appointment.
Please be punctual for your appointment, and if you are unable to keep your appointment please let us know as soon as possible, so we may use it for someone else. If you have an urgent medical problem that will not wait for a routine appointment please tell the receptionist.
All new patients will be asked to complete a registration form. An appointment will be made with either one of the doctors or a practice nurse when more details of your medical history can be taken, and any further care necessary planned.
To make an appointment by telephone – please ring your usual surgery during surgery hours.
We do not offer specific telephone consultations, but staff are happy to deal with telephone enquiries wherever possible.
If the doctor or nurse is unable to speak to you at the time you phone, then we will take a message and the receptionist will try and give an indication of when they will get back to you. In most situations that will be within 24 hours.
For urgent advice there is always a duty doctor on call, who will be able to get back to you quickly if required.
Only ask for a home visit when it is really needed. Please remember to call before 10.00am and give as much information as you can. This allows the doctor to visit the most urgent cases first and avoids unnecessary delays.
Between 6.30pm in the evening and 8.00am in the morning, and also at weekends, the Derbyshire County Primary Care Trust is responsible for the out of hours service. However, if in an emergency, you require a doctor outside surgery hours, please ring your usual surgery, from where help can be accessed.
If you need regular prescriptions and your doctor does not need to see you every time, he will authorise a repeat prescription.
Please retain the printed sheet attached to your last prescription. This will show the drugs you are taking and the total number that will be issued before your treatment needs to be reviewed.
We can now accept online orders for repeat prescriptions .
Requests for repeat prescriptions can also be made by telephone, in writing, or by calling the prescription line at the surgery where you are registered.

When telephoning please try to have your most recent printed sheet to hand. Please allow two complete working days before collection. Where possible give the exact drug name as written on your sheet when ordering.
Does your repeat prescription have items on it that you are no longer taking? If so, please point this out to a member of staff who will arrange to remove it where appropriate.
Please notify us immediately of any change in your name, address or telephone number. We should always have your current address and telephone number, as we may need to contact you urgently.
We are always pleased to receive suggestions for improving our services, and we also like to receive compliments as well.
The practice operates a Practice Complaints Procedure, as part of the NHS System for dealing with complaints, which can be made either verbally or in writing.
If you feel you wish to complain or make a suggestion about any aspect of our service, or of that elsewhere in the NHS, our Practice Manager, Stephen Horton, will be pleased to give you any information you require. In addition, we have a practice complaints leaflet which gives details of the procedure and is available from reception.

Our aim is to give you the highest possible standard of service and we try to deal swiftly with any problems that may occur.
Patients with minor injuries who need medical attention should be taken to the Casualty Department at the Whitworth Hospital. More serious injuries should be taken to the A & E Department at Chesterfield Royal Hospital.

With regard to travel immunisations / vaccinations, kindly make an appointment at least eight weeks in advance of your holiday to ensure adequate cover. A charge may be made for certain vaccinations and drugs that are not covered by the NHS.
Do you look after someone who has a disability or long term illness and couldn´t manage without your help? If you would like some help and support, please ask at Reception, or ring the Derbyshire Carers Association on 01773 743355.
One Wednesday afternoon of each month (except August and December) the practice will be shut from 12:30pm. This allows us to provide ongoing training for all staff.
The out of hours service (Derbyshire Healthcare) will take all calls and manage requests for urgent review. Unfortunately we cannot offer a routine appointment or prescribing service at these times.
